Grassroots Labs is an Online Marketplace for Blood Testing
Our goal is to get Medi-Share Members who participate in the Health Partnership Program, the lab tests they need as quickly and affordably as possible.
In this guide we will show you how to:
Register for an Account and Purchase Your Tests
Find & Visit the Nearest Test Center to Your Location
Have Your Results Sent to Medi-Share Directly

STEP 1
Let Us Send Your Results to Medi-Share so You Don't Have To!
Medi-Share Release of Information
Sign our simple, online release of information before purchasing your tests, and we will take care of sending your results to Medi-Share on your behalf, when they become available (3-4 business days).
PLEASE NOTE:
If you have already signed this Release of Information for a previous order, you may skip this step.
STEP 2
Find a Testing Center Near You
Grassroots Labs works with Quest Diagnostics to provide over 2000+ locations across the United States to have your lab tests performed.
Clicking the button below will open a new window where you can search for a testing center close by using your zip code.
Don't worry about remembering those locations. When you complete your order on the Grassroots Labs marketplace, we will email a list of the three closest testing centers to your location with their contact information.
When finished locating a testing center nearby, just come back to this guide and scroll down to move through the process. :)
STEP 3
Register Your Account
To get started with Grassroots Labs, you will need to register for an account. Each person who will need testing will have to create their own, separate account.
Watch the tutorial video to the left to be guided step by step.
When you are ready to create your account, click the button below and then click "Register" at the top of the Grassroots Labs website.
Please Note: If after you register for an account, you try to log in and the system does not go anywhere, please check your email for your "Account Activation Email" and click the link inside to activate your account. Once activated, you will be able to checkout.
STEP 4
Purchasing Your Tests
Once you have created an account with Grassroots Labs, you can add your Lipid & Hemoglobin A1c Panel to your cart and check out.

STEP 5
Print Your Lab Order / Requisition
After purchasing your tests, you can print out your requisition, a.k.a. your lab order to take with you to the testing center.

STEP 6
Visit the Testing Center
Now that you have purchased your tests and printed out your requisition, you are ready to visit the testing center to get tested.
A few things to remember:
Take your printed requisition to the testing center
Check the contact information for your testing center and make sure they are open when you plan on visiting.
Remember to fast the night before going to have your tests performed. No food or liquid EXCEPT water. Drink lots of water.
Congrats! You are Finished!
If you signed our Release of Information at the beginning of this guide, we will send your results to Medi-Share on your behalf.
